What is it:

This service provides non-medical support to individual who live in a home setting.

What We Do:

We Provide person-centered services to individual with Developmental Disabilities creating SMARTER plans of care to achieve each individual’s goals and dreams.

What kind of Supports are Provided?

Our dedicated Direct Support Professionals (DSPs) may provide the following services:

  1. Basic personal care including bathing, grooming, dressing, and assistance with toileting;
  2. Assisting with incontinence, including bed pan use, changing urinary drainage bags, protective underwear etc;
  3. Assisting the individual with transfer, ambulation, and exercise as prescribed;
  4. Assisting the individual with self-administration of medication;
  5. Observing, recording, and reporting the individual’s physical condition, behavior, or appearance;
  6. Assisting the individual with meal preparation in accordance with dietary guidelines, and assistance with eating;
  7. Assistance to implement universal precautions to ensure infection control;
  8. Assistance with tasks related to keeping the individual’s living area in a condition that promotes the individual’s health and comfort;
  9. Assisting the individual with transportation to medical and medically related appointments;
  10. Assisting the individual to his or her place of employment;
  11. Assisting the individual with shopping for items related to promoting the individual’s nutritional status and other health needs;
